Instructions: Complete the required information below. Carefully review the information provided for accuracy. Click the "Submit Registration" button and you will then be prompted for payment of the required processing fee. You must appear in person within 30 day's of submitting your registration to be fingerprinted with a valid photo ID and copy of your paid receipt. Registration is nonrefundable or transferable. Applicants and licensees who have been provided fingerprint cards by their perspective employer or licensing agency should bring these cards with them when arriving to be fingerprinted. If necesary, fingerprint cards may also be obtained in our office.
